Job summary To act as assessment co-ordinator for Medical division within CHFT. Supporting the division in maintaining and supporting the clinical assessment service. Working closely with Clinicians, Secretaries, Managers and admin staff, to enable patient pathways to be dealt with in a timely manner and to be the point for contact for divisional service users.
